PROBLEM TO BE SOLVED: To obtain a clean information output without generating a toner stain by preventing an adhesive from being transferred to a drum of a printer by dealing with at a label sheet side. SOLUTION: The method for forming a label sheet comprises the steps of releasably connecting a label base material 7 to a rear surface side of a fill-in cut form 6 corresponding to a part except a region 11 to be perforated of cutting perforations 5 of the cut form 6, and perforating the cutting perforations 5 at the region 11 of the cut form 6.
