PROBLEM TO BE SOLVED: To provide an image forming device constituted so that the image of an object extended long and narrow can be effectively formed. SOLUTION: This image forming device, for forming the image of the object 5 so that the image 6 formed by the image forming device shows substantially identical size to that of the object 5 whose image should be formed, is constituted by including at least four optically functioning surfaces 8, 10, 11, 12, 13 or 14 contributing to an image forming action. All of the optically functioning surfaces 8, 10, 11, 12, 13 and 14 are provided with a cylindrical lens or a shape being similar to the cylindrical lens. At least four optically functioning surfaces 11, 12, 13 and 14 are provided with substantially mutually facing parallel cylindrical shafts. Moreover, the image forming device is provided with at least one surface 8 or 10 optically functioning further and contributing to the image forming action and the cylindrical shafts of the surface 8 and 10 are arrayed almost vertically to those of at least four surfaces 11, 12, 13 and 14 arrayed in parallel.
