PROBLEM TO BE SOLVED: To obtain an address label with a prize contest application ticket which improves the efficiency of business transaction by devising the address label so as to impart motivation to have a mail medium for sales promotion unsealed and the contents of the sealed object viewed on the receiver side as far as possible so as to enhance the unsealing rate of the direct mail, etc., when the mail medium is sealed and is forwarded particularly by a direct mail, etc., and to allow the mechanical easy check of the receiver when a reply postcard is received from the receiver by using the sealed mail medium, such as the reply postcard. SOLUTION: The one block portion on the front side of a separably segmented slip form is formed as an address certificate form having a receiver address entry part and the other block portion is formed as a prize contest application form displayed with prize contest application information. The rear side of the slip form is successively provided with an adhesive layer and release paper for coating the adhesive layer. A release layer is disposed between the prize contest application paper piece and the adhesive layer.
