PURPOSE: An apparatus and method for molding an anchor bolt hole to concrete structure is provided to mold an anchor bolt hole using a removable divided sleeve. CONSTITUTION: The apparatus for molding an anchor bolt hole comprises a plurality of divided sleeves having a same shape to form a cylindrical shape when the sleeves are connected, wherein, an inwardly bent inner contact surface is provided on an end of an outer surface of the sleeve, and spiral rods which are protruded upwardly in different amounts are welded between the respective outer surfaces and the inner contact surfaces; a lower cap(50) having stepped jaw(52) in which lower end of an outer side surface of the divided sleeve is rested, and a connecting hole(56) into which a lower end of a draw rod(54) is connected, wherein, the stepped jaw is formed around the rim of the upper surface, and the connecting hole is provided in the center of the upper surface; and an upper cap(60) having a center hole(62) into which the draw rod is inserted, a plurality of outer holes(64) into which the divided spiral rods are inserted, and groove(66) in which an upper end of the outer side surface is rested.
