PROBLEM TO BE SOLVED: To adjust positions between an optical system lens as an optical means and an imaging device as a solid-state image sensor in each three-dimensional direction easily and precisely. SOLUTION: A plurality of notches 40a-40a are respectively provided on an imaging substrate 40 where the solid-state image sensor 41 is disposed. Protruding parts 30b-30b are respectively provided on an illuminating substrate 30 where the optical means 42 is disposed. The relative position and the mounting direction of the illuminating substrate 30 to the imaging substrate 40 are adjusted by engaging the protruding parts 30b-30b with the notches 40a-40a, and the illuminating substrate 30 is fixed on the imaging substrate 40.
