The invention concerns a perforating template for a dental implant, in particular a dental implant having at least a bore designed to receive a clamp. It enables the perforating instrument to be guided for producing one or several lateral holes in the bone for passing through one or several clamps. It likewise guides the fixing of said clamps. It consists of a template body (9) accurately fixed on a positioning member (1) integral with the implant (14), and an elongated part (12) or socket which slides in the template body (9) to be urged in contact with the bone (13). It enables good adaptation to anatomical variations of the bone and avoids the displacement of the template body for making several holes in the bone.
