PROBLEM TO BE SOLVED: To reduce the weight of a concrete form for building an anchor portion of a steel tower having a circular-arc mold surface, to make the installation easier, and to enhance workability and safety. SOLUTION: A circular arc type mold surface portion 2 equipped with a predetermined curvature is provided on the inner side surface of a synthetic resin form 1 formed in to a circular arc shape using polyethylene or polypropylene as a raw material, and a ring-shaped rib 6 and a center boss portion 7 with the end surfaces mutually coincided with the same plane perpendicular to the center of the circular arc surface inside a square surface 4 enclosed by each peripheral edge portion 3 of four sides and a longitudinal middle rib 5. Scaffold members separate from the outer enclosure side are made attachable to a screw hole of an attaching fittings 9 inserted to the center boss portion. In this way, forms for concrete to be installed at multi-stages sloped enclosing the anchor portion can be easily constructed, and a clean concrete placement surface, excellent in workability and safety can be obtained.
