PROBLEM TO BE SOLVED: To use an inexpensive net small in wire diameter as a side form in an embedment form for forming a slope frame provided with a spacer for connection, and to easily implement various works from the side, for example, a work to bind a lower end main bar to a lower side wire of the space for connection, a work to bind the lower end bar to a stirrup, and a work to fill mortar in the form. SOLUTION: A folding and a self-supporting a frame body 1 is formed by pivotably connecting a plurality of spacers 6 for connection comprising winds 7a, 7b, 8a, (8c) to a pair of side frames 5 formed in a rectangular shape of a plurality of transverse wires 3a, 3b, 3c, 3d and longitudinal wires 4a, 4b, 4c. A rectangular and net-like side form 2 is mounted on the side frames 5 in an attachable/detachable manner, or in an oscillating manner with one side edge in the vertical direction as a supporting point.
