PROBLEM TO BE SOLVED: To prepare a document whose information is easily inputted to a computer by describing information described in a format that can be recognized by a person and a code in a format that is easily inputted to a computer on the same paper and providing a place where the seal or the like is applied. SOLUTION: In a document being a subsidy application form, the genuineness of the document is confirmed at the same level as that of a normal document, based on the signature of the person concerned and the seal 2. A code that is easily inputted to a computer is also described at the lower left part of the paper on which the signature and the seal 2 are performed. Thus, an inputting person can easily input the information to a computer after visually confirming the application form and reduce errors in inputting the information while reducing job labor. |
