PROBLEM TO BE SOLVED: To enable the smooth execution of a surgery and to prevent the erroneous recognition toward the celom by neglect in changing over of an endoscope image by enabling the changeover to the endoscope image converted to an inverted image or mirror image without manipulating an image processing means. SOLUTION: A camera head 30 as a detecting means for detecting the insertion direction of an endoscope 10 into a subject for an observer for observing the endoscope image displayed on a monitor 70 is provided with a light emitting part 51. A detector 50 is provided with a light receiving part 52 for receiving the light from the light emitting part 51, by which the direction of the endoscope 10 is detected without contact. The image processing device 60 is constituted by providing the device with an image processing means for converting the image signal from a CCU 40 to the inverted image or mirror image and a changeover means for changing over the video signal from the CCU 40 and the image signal converted by the image processing means in accordance with the detecting signal of the detector 50.
