PROBLEM TO BE SOLVED: To upgrade printability even with a label cut form having a label portion by suppressing formation of a rugged part on the portion. SOLUTION: This label cut form comprises a label portion 8 partly on a print sheet 2 describing information on its front surface. In this case, the label portion 8 has a label 9 partitioned at a cut-out part 3 in the sheet 2 for printing, and a tack sheet 10d disposed on a rear surface side of the label 9 and releasably covering an adhesive 12 provided on the rear surface of a base material 11 with a release sheet 13. Further, the label 9 is adhered to a tacky sheet 10 through an adhesive 14 disposed between the label 9 and the material 11.
