PROBLEM TO BE SOLVED: To permit electrical connection by connecting an actuator to a base fixedly and electrically by fixing a power supply member and a terminal member. SOLUTION: Relative position adjustment between an actuator and a base 11 is performed, power supply members 7a-7d provided in a fixed member 8 and terminal members 13a-13d provided in a base 11 are fixed mutually using a conduction member such as solder and the like. A current driving the actuator is supplied to a focus coil and a tracking coil through a wiring pattern 12 on the base 11, terminal members 13a-13d, power supply members 7a-7d on the fixed member 8. As fixing of the actuator and the base and connection for supplying power can be performed simultaneously, assembly can be simplified, a flexible substrate for supplying power to the actuator is not required, manufacturing cost can be reduced.
