PROBLEM TO BE SOLVED: To accurately identify the types of slips by comparing the type of a read slip and the mark position of image data with those which are stored in a storage means and detecting the position shift of the image data. SOLUTION: A form 10 includes three marks 11, 12 and 13 and the distances are set at w1 and h1 between the marks 11 and 12 and the marks 11 and 13 respectively. Meanwhile, a form 20 includes three marks 21, 22 and 23 and the distances are set at w1 and h2 between the marks 21 and 22 and the marks 21 and 23 respectively. Thus, the forms 10 and 20 can be identified since only one of both distances of marks (the distance between the marks 11 and 13 and the distances between the marks 21 and 22) is different between both forms. As a result, the types of slips can be accurately identified.
