<p>PROBLEM TO BE SOLVED: To eliminate such trouble as to copy the described items from an ID card and to enter them in a sheet, and reduce labor of making out a document when ID cards of various kinds are renewed and issued. SOLUTION: A document preparing apparatus 100 for preparing a document for the issue of an ID card includes an information reading means 11 for reading image information on both front and rear surfaces of an ID card 30, an image forming means 12 for forming an image on a predetermined sheet on the basis of image acquision information by the information reading means 11, and a controlling means 14 for controlling the image forming means 12 so that images on the front surface of and/or the rear surface of the ID card 30 are formed in the predesignated region with respect to the sheet. With this constitution, an application for renewal or the like can automatically be made out when the ID card 30 is renewed and issued. Accordingly, the apparatus can dispense with such trouble as to copy the described items from the ID card 30 and enter them in a sheet.</p> |
