The smart card carries patient identification, and information regarding treatment and vaccinations etc. The procedure for recording medical information includes a computer (1) in the surgery and a program (2) running during a patient's consultation. The patient is provided with a smart card with a chip (3) which includes identification information, and the computer has a smart card reader. The card is inserted into the reader to enable the information to be read and memorized, and is then compared with that provided by the program running on the computer. The computer is used to record the treatment or vaccinations provided for the patient, and this may be used to print a certificate record (10). The information is subsequently recorded both within the program of the computer, and on the smart card kept by the patient.
