PROBLEM TO BE SOLVED: To manage the state that a defect area has not been replaced by a replacement area by determining the value of state information indicating whether the defect region has been replaced by the replacement area or not, and recording a defect management information including the value at a defect management information region. SOLUTION: A state field 22a is used to indicate whether a defect sector has been replaced by a replacement sector or not. The position of the defect sector is expressed, for example, by the physical sector number of the defect sector, while the position of the replacement sector is expressed by the physical sector number of the replacement sector. For example, when the value of the flag 22a-1 is 1, it indicates that the defect sector has not yet been replaced by the replacement sector. On the other hand, when the value of the flag 22a-1 is 0, it indicates that the defect sector has been replaced by the replacement sector. Thus, the value of state information indicating whether the defect area has been replaced by the replacement area or not is determined, and it is stored in the defect management information area.
