PURPOSE: A color patch for a color correction is provided to obtain one sheet color patch having multiple patch regions. CONSTITUTION: A digital correction(7') of a color correction is formed of a shadow correction portion(71), an achromatic color portion(72), and a color correction portion(73). A shadow correction, achromatic adjusting and color correction are performed with respect to a digital CCD output outputted from three optical conversion devices(4R, 4G and 4B) and converted into a digital value by an analog correction system(5) and an analog/digital converter system(6). The color correction signals(Ro, Go and Bo) obtained by the apparatus are converted into the type of the data transferred to the printer output unit based on the image output apparatus or are processed into binary code values for a facsimile transmission. The sheet includes a document recognition hole, a white patch region, a patch region, a color patch region, and marking holes for recognizing the end portions of the patch region and the color patch region.
