The housing has steam holes (15), and an inner, separate or integral, instrument-holder (17) made of the same material as the housing. The instrument-holder has attachments (24) for a range of instruments (26). One of the housing parts has a control (9) positioned in a rotary apart (6). A ratchet appliance (11) restricts the torque transmitted from the housing part to the rotary part to a maximum.
