PROBLEM TO BE SOLVED: To provide a delivery slip form without miss matching in a fill-in item of an attached slip form from a content of a delivery article and without being known by another person for the fill-in item of a detailed slip form for the content of the article. SOLUTION: In the delivery slip form 1 comprising a laminating slip form 2 having a laminating part on a rear surface side and distribution information fill-in parts 12, 13 on a front surface side, a receipt slip form 3 having delivery information fill-in parts 19, 20 and a receipt impression part 21 on a front surface side, a detailed slip form 4 similarly having a delivery article detailed information fill-in part 24 on the front surface side to be continuously connected, front surface sides of the forms 4 and 3 can be folded up at a folding and cutting sew line 6, an adhesive part covering a release tape 28 with an adhesive to be bondable in place to superposed oppositely contact surface is provided to maintain the superposed states of both the forms 3, 4 when the forms 3, 4 are superposed, and a cutting sew line 26 is provided to be separable after adhering.
