PROBLEM TO BE SOLVED: To shorten time after an issuing switch is pressed until a voting form is issued and to efficiently issue the voting form. SOLUTION: A discharge carrying path 52 is branched from the carrying path 49 of the voting form and arranged, and a changeover lever 54 is provided on a branching part. Also, in the middle of the carrying path 49, an abnormality detection sensor 56 for detecting carrying abnormality such as double feed and the abnormality of paper itself is arranged. Then, when the abnormality is not detected in the abnormality detection sensor 56, the voting form (p) is carried to a standby position where a tip reaches between the changeover lever 54 and a voting form feeding port 48 and is made to stand by for a switch operation for issuance. Also, when the abnormality is detected, the changeover lever 54 is switched to the side of the discharge carrying path 52, the voting form (p) is discharged and a new voting form (p) is sent from a hopper 41 into the carrying path 49. Then, a similar operation is repeated and the setting of the normal voting form to the standby position is completed before the switch operation of the issuance.
