PROBLEM TO BE SOLVED: To realize a questionnaire preparing/collecting system utilizing an advanced intelligent network and capable of easily preparing and collecting questionnaires on a home page without purchasing a system for adding up questionnaires through a telephone. SOLUTION: In the questionnaire preparing/collecting system having a clearly separated exchanging function and a service control/management function and utilizing a network, questionnaires are prepared by accessing, through the internet, a home page for questionnaire preparing/collecting service provided by a Web server in a service management system 50 in the network, the prepared questionnaires are connected to an optional questionnaire target (replier) 10 by using the network, the replied questionnaires are collected and the collected result is transferred to the Web server.
