PROBLEM TO BE SOLVED: To provide a delivery form wherein a delivery tag and a store copy are prevented from peeling off from the surface of an adhesive sheet even when a printing output is performed by a printer while bending the delivery form, or the delivery form is affixed on a cylindrical article to be delivered, or an article to be delivered of which the surface is a curved surface, or the like. SOLUTION: This delivery form has a form paper 3 which is separatably partitioned into a form having a delivery information listing section 11 at least on the surface side, and other paper pieces, an adhesive layer which is formed on the rear surface side of the form paper 3, and a release paper 6 which covers the rear surface side of the adhesive layer, and the form 3 and the adhesive layer are separatable, and in such a delivery form 1, the form paper 3 is partitioned by slits 9a and 9b, and on at least one 9a of the slits, a connected section 10 is provided.
