PROBLEM TO BE SOLVED: To provide a delivery form which prevents a form from accidentally being peeled from an adhesive layer when a release paper is peeled from the rear surface of a form paper. SOLUTION: This delivery form has a form paper 3 which is separatably partitioned into a form having a delivery information listing section at least on the surface side, and other paper pieces, an adhesive layer which is formed on the rear surface side of the form paper 3, and a release paper 6 which covers the rear surface side of the adhesive layer, and the form and the adhesive layer are separatable, and in such a delivery form 1, at a section wherein the form 1 is adjacent to other paper pieces, a peeling end section 11a which becomes the beginning of a peeling of the form 1 is provided.
