PROBLEM TO BE SOLVED: To mold a concrete product without difference from a form shape and to save a labor hour of reassembling an elastic mold member with a backing member by simplifying manufacture of the form and preventing a deviation or a separation of the mold member. SOLUTION: The form comprises an elastic mold member 4 of a thermoplastic elastic material 40 formed at its inner surface on a mold surface 41, and a backing member 6 supporting the mold member by being placed in the state covering an outer surface of the member 4 and then solidifying it. The method for manufacturing the form comprises the steps of placing a solidifiable flowable material (ready-mixed concrete 60) in the state covering the surface of the member 4, and solidifying the material 60, thereby forming the backing member 6 brought into integrally close contact with the member 4.
