PROBLEM TO BE SOLVED: To form a notch hole easily and accurately in the inside of a concrete product. SOLUTION: A hole form 20 having almost the same outer shape as a notch hole 2 is arranged at a corresponding position for forming the notch hole 2 on the concrete forming side of a inside form 11, in a freely attaching/detaching manner and in such a manner that the hole form 20 is not movable when the concrete is formed. After the separation of the inside form 11 from the set concrete product, the hole form 20 is separated. For this purpose, the hole form 20 is attachable to and detachable from a form 10 by a support device 30 arranged inside the form 10. A shear connector pin hole is made, as a support device 30, on the form mounting side of almost the center part of the hole form 20 and at the same time, a pin insertion hole 12 is made almost at the center position of the corresponding position for forming the notch hole 2 of the inside form 11 of the form 10. Thus the hole form 20 is positioned by inserting/drawing a shear connector pin 34 in/from the shear connector pin hole 24.
