PROBLEM TO BE SOLVED: To obtain the information recording medium which is reduced in the recording area of address marks by recording a code sequence for track number identification by a specific recording method and the information recording and reproducing device which can efficiently record information by using the medium. SOLUTION: The information recording medium is a medium which has address marks including the code sequence for track number identification recorded at its address mark recording part; and part of the code sequence for track number identification is divided and recorded at another address mark recording part 2. The high-order digits of the divided code sequences are recorded at address mark recording parts 2 on different recording surfaces and further the low-order digits of the divided code sequences are recorded repeatedly. The information recording and reproducing device has a code composing function of composing a code sequence for track number identification by using address mark read signals read by respective heads.
