PROBLEM TO BE SOLVED: To provide a hair care counseling sheet which is composed of a combination of an information sheet, showing useful counseling information for a customer, which is to be handed to the customer after counseling and a diagnostic sheet which a counselor keeps in custody with an accessible range and uses for next counseling. SOLUTION: This hair care counseling sheet is used when a cosmetic salesperson or a hair care specialist extends hair care counseling services to the customer. In this case, the sheet has a commodity information display part showing information relative to plural different kinds of cosmetics for hair care treatment and a massage information display part showing information relative to plural different kinds of a scalp massage method. In addition, an information sheet A to be handed to the customer after counseling is provided and further it has a column for entering the name of the customer, a column for entering an answer concerning the state of hair and a column for entering an answer concerning the state of a scalp and the sheet has a diagnostic sheet B to be kept in custody by the cosmetic salesperson or the hair care specialist within an accessible range after counseling.
