PROBLEM TO BE SOLVED: To examine eyes smoothly by easily obtaining information on the contents or usage of examination optotype. SOLUTION: This optotype for examining the function of a subject's eye has an examination optotype plate 3, box to store the optotype plate 3, and an optotype display changing means. The optotype plate 3 has a first surface with plural optotype display parts in which different optotype is displayed and a second surface with an information visual display part in which information on the optotype is displayed. The box to store the optotype plate has an index window for presenting one of the plural optotype display parts and an information window 7 for confirming index information 15a-15j at least on the presented examination index. The optotype display changing means moves a required optotype display part to the optotype window.
