PURPOSE: A picture-attached certificate is provided to maintain itself as a memorial of a prize winner as well as to make people feel curious or interested by seeing the face of the prize winner or his usual appearance. CONSTITUTION: A picture-attached certificate is composed of: a title column(10) recording a prize title; a name column(20) recording a name of a prize winner; a prize particular column(30) describing the result or the prize contents and the like of the prize winner; an organization record column(40) for authenticating its authority by recording an organization which gives the certificate; and a picture attach column(50) able to show the face or the appearance of the prize winner by attaching his picture.
