PROBLEM TO BE SOLVED: To switch illumination modes appropriately by a simple rotating operation, to lighten the burden imposed on the eye for example by soft diffused light, and to enable observation of minute portions of a localized area by bright spotlight narrowed locally. SOLUTION: This lighting system is provided with a substrate 3 rotationally provided to the tip of an arm 1, a light source immovably fixed to the substrate 3, and a cover 5 rotatable to the substrate 3 and equipped with an irregular area having minute irregularities being frosted glass on the under surface from which the light from the light source outgoes and a transparent area provide to a part of the irregular area, and a handle 6 mounted integrally with the cover 5 and gripped by a patient and a health care atleudant. When the cover 5 is rotated to position the irregular area on an outgoing light path from the light source, diffused light is formed. When the cover 5 is rotated to the other portion to position the transparent area on the outgoing light path from light source, spotlight is formed.
