PROBLEM TO BE SOLVED: To prevent multiple interference occurring between end surfaces of an optical element and an optical fiber opposite to each other. SOLUTION: A V groove 2 is formed on one side peripheral part of a semiconductor substrate 1, and the end part of the optical fiber 3 is fixed to the V groove 2. The V groove 4 is formed on the other peripheral part parallel to the V groove 2, and the end part of the optical fiber 5 is fixed to the V groove 4. The optical element 6 such as an optical modulator, an optical amplifier provided with an optical waveguide 6a is arranged on a prescribed position between the V grooves 2, 4, and an emission beam P1 from the end surface of the optical fiber 3 is made incident on one side end surface of the optical waveguide 6a, and the beam P2 passing through the optical waveguide 6a and emitted from the other side end surface is made incident on the end surface of the optical fiber 5. The end surface of the optical fiber 3 and one side end surface of the optical waveguide 6a opposite to each other are made nonparallel, and further, the other side end surface of the optical waveguide 6a and the end surface of the optical fiber 4 opposite to each other are made nonparallel.
