PROBLEM TO BE SOLVED: To easily and freely cope even with addition and change in an item name and a condition, when the item name and the condition are extracted and data retrieval is performed. SOLUTION: A layout of a slip is prepared in a table format on a sheet by a layout preparation means 15, an item name and plural conditions are extracted and set on the data base on a data base by an item condition set means 18, the data base is retrieved by the item name and the plural conditions extracted by a retrieval execution means 19, and the slip on which the retrieval result of the data base is written is outputted by a slip output means 20.
