PROBLEM TO BE SOLVED: To simplify the optical axis adjustment work of an optical waveguide and a film carrier to the optical axis of an image forming optical system. SOLUTION: A supporting positioning plate 100 is tightened and fixed to a base frame 102 by four bolts 120, supported to a prescribed position in an optical axis direction by the base frame 102 and made movable in an optional direction orthogonal to the optical axis L by turning the four bolts 120 to a loosened state. To the supporting positioning plate 100, a mirror box 30 and the film carrier 32 are respectively mounted. At the time, the mirror box 30 and the film carrier 32 are respectively positioned to prescribed mounting positions by the supporting positioning plate 100 and the center light beam of light emitted from the mirror box 30 passes through the width direction center of a photo film supported by the film carrier 32.
