DISK-SENDING MEDIUM AND MANUFACTURE THEREOF

PROBLEM TO BE SOLVED: To provide a disk-sending medium wherein packaging can be performed surely and at low cost by means of a simple construction and address information or individual information can be directly printed and moreover both the address information and individual information can be matched with each other. SOLUTION: A medium base material which can be folded at least in two and in which an incision 11b into which a disk 20 can be inserted is formed and a shrink film which covers the opposite surfaces of the disk by being thermally shrunk under such conditions that the medium base material is folded back are provided. A front surface side of the folded medium base material where no incision is formed serves as an address information printing part X, and a side of the medium base material which has therein an incision 11b and is covered with the disk under such conditions that the disk is inserted into the incision serves as a concealed information printing part Z.
