Improved laser keratotomy is provided by defining a pocket in a cornea (16) and insertion thereinto of a spatula (24) or cutting blade having thereon reflective surface (35), or opening means (46) for controlling application o f a laser beam (12) to corneal tissue in anterior or posterior pocket walls to control and define areas of ablation of corneal tissue.
