PROBLEM TO BE SOLVED: To systematically check the causes, analysis and recurrence preventing measures of human errors in various sections such as design and manufacture. SOLUTION: In a human error evaluation table, factors (B) causing errors are horizontally regulated on respective columns as subject (B-1), hardware (B-2), software (B-3), environments (B-4), person (B-5), and management (B-6) and preventing measures are vertically regulated on respective lines as six items consisting of removal (1), limitation (2), reduction of recognition load (3), reduction of physical load (4), detection (5), and ease of influence (6). When a human error occurs, the contents of the error are written in an error mode line, whether the error corresponds to each of the factors B-1 to B-6 or not is checked and a corresponding factor is extracted. Whether the extracted factor corresponds to each of items (1) to (6) in preventing measures or not is successively checked and a corresponding preventing measure is written. |
