PROBLEM TO BE SOLVED: To simply correct a questionnaire item to content that corresponds to a patient by reading corresponding questionnaire data based on a patient data and outputting it as a questionnaire form after making the questionnaire data correspond to the patient code and storing it. SOLUTION: An input device 3 inputs questionnaire data. A storage device 1 makes the questionnaire data inputted by the device 3 correspond to a questionnaire data and stores it. A display device 2 shows the questionnaire data. An output device 4 outputs the questionnaire data. A controller 5 shows the questionnaire data on the device 2 and inputs a questionnaire item of the questionnaire data for elimination, addition or change with the device 3, makes the questionnaire data correspond to a patient code, stores it in the device 1, after that, reads corresponding questionnaire data based on the patient code and outputs it as a questionnaire form with the device 4. |
