PROBLEM TO BE SOLVED: To provide a multiple hologram information recording medium for reproduction only which is low in cost and has large capacity. SOLUTION: In a reproduction only multiple hologram information recording medium in which single mode planar optical waveguides are laminated, at least one of a core layer 12 and a clad layer 11 in respective waveguides is provided with a periodic scattering factor having almost same period as the period of a waveguide mode, and waveguided light 16 in the waveguide is diffracted to the outside of the waveguide by the periodic scattering factor to form a hologram image 20.
