A pacemaker comprises stimulation threshold measuring means for measuring the stimulation threshold voltage value, a means (8, 14) for generating a safety margin, and a pulse generator for delivering stimulation pulses of variable amplitudes and duration. The pulse generator is controlled to deliver stimulation pulses of amplitudes related to the measured threshold value and said safety margin. The pulse generator is thus controlled to deliver stimulation pulses of amplitudes (Ustim) which are varying with varying values of the measured threshold value (Uthres), such that said safety margin (m) is automatically changed in accordance with a predetermined relationship between said safety margin and said measured threshold value, wherein said relationship being such that said safety margin is progressively increased when the measured threshold value is increased.
