A microsurgical lighting instrument (50) includes a hollow needle (30), and a light guide (40) received coaxially within the hollow needle (30), and connected to a light source for transmitting light to a surgical site of a patient. The light guide has a distal end (43) formed with an end face of circular segment shape (42), defined by a plane oriented orthogonal to the axis of the hollow needle (30), a slanted surface (44) adjacent the end face (42), and extending rearward at an acute angle, with the slanted surface (44) being lined by a coating for reflection of incident light rays.
