Disclosed is a cardio-protective agent for use in prophylaxis and treatment of myocardial disease and/or myocardial cell injury, which comprises an isoquinolinesulfonamide derivative represented by the following formula or a pharmaceutically acceptable acid addition salt thereof (see fig. I) wherein R1 represents H, Cl or OH; each of A, R2, R3 and R4 is selected depending on the type of R1, wherein A represents C2-C6 alkylene, which is optionally substituted with C1-C10 alkyl, cinnamyl, phenyl or benzyl, R2 represents H or cycloalkyl having 6 carbon atoms or less, R3 represents H, straight or branched C1-C6 alkyl, cinnamyl, phenyl or benzyl, or R2 and R3 together form alkylene having 4 carbon atoms or less, which is optionally substituted with C1-C10 alkyl, phenyl or benzoyl; and R4 represents H, straight or branched C1-C6 alkyl, phenyl, benzyl, benzoyl, cinnamyl, cinnamoyl, furoyl, (see fig. II) (see fig. III) or , or R3 and R4 together with a neighboring nitrogen atom form or 6-membered heterocyclic group, which optionally contains an oxygen atom; R5 represents straight or branched C1-C6 alkyl; each of R6 and R7 is selected depending on the type of R1, wherein each of. R6 and R7 independently represents H or CH3, or R6 and R7 together form C2-C4 alkylene.
