PROBLEM TO BE SOLVED: To effectively manage information useful for a patient, a medical facility or a caring person by providing an explanation degree entry section for writing the explanation degree of condition to a patient him/herself and his/her family in a medical recording note. SOLUTION: The medical recording note 50 registered in a group diagnosis and medical treatment database 21 is provided with an explanation degree entry section 51 for writing the explanation degree of condition to the patient him/herself and an explanation degree entry section 52 for writing the explanation degree of condition to his/her family. This note 50 can be referred to from terminal devices 11a to 16a capable of accessing the database 21 through a network 40. Thereby, when a patient who received medical treatment at a family doctor's office 11 is hospitalized in a hospital 14, a doctor in the hospital 14 can deal with a suited to the patient and his/her family based on the explanation degree of condition until then by referring to the note of this patient. As the result, the taking-over processing is unnecessitated to improve the efficiency of hospitalization procedure.
