System for recording medical, insurance and welfare information

A novel information management system for utilizing medical, insurance and welfare information handled in a medical institution such as a hospital, a pharmacy, or a medical examination institute. A write once recording medium is employed for storing information concerning the medical record and carried by a patient. An information writing means for storing medical information on the information recording medium carried by the patient is installed in each medical institute or at each section thereof. Each medical institute or each section thereof can write information on the recording medium carried by the patient or reads information therefrom within a range which the operator is allowed.
