PROBLEM TO BE SOLVED: To obtain a pharmaceutical composition obtained by combining a substantially pure diastereomer of a tetrahydrophorate derivative with a pharmaceutically acceptable carrier. SOLUTION: This therapeutic preparation composition obtained by separating an equivalent weight mixture of (6S) and (6R) diastereomers by utilizing different solubility of the (6S) diastereomers in a polar solvent, and usable as a rescue agent for methotrexate (a) is a therapeutically acceptable compound containing >=95 wt.% 5-formyl-tetrahydrophrate calcium in a form of (6S) diastereomer with remaining <=5 wt.% (6R) diastereomer of 5-formyl-tetrahydrophrate calcium, (b) is regulated so that the amount of an acceptable compound may be at least 10.4 g and (c) has a form of an effective amount for multistep cure of 25-150 mg of the compound limited to the limitation (a) combined with a human acceptable carrier for treating.
