PROBLEM TO BE SOLVED: To provide an ultrasonic trocal system which is good in insertability and has high safety. SOLUTION: An ultrasonic wave transmission member 5 connected to a horn is inserted into a trocal tube 2 via a convergent adapter 6 provided with a convergent part 6a at its front end. The convergent part 6a of the convergent adapter 6 is provided with a first tapered part 61 which is formed with a taper angle to make the outside diameter gradually smaller than the outer periphery diameter size toward the front end atθ1 and a second tapered part 62 which exists nearer the front end side than the first tapered part 61 and is formed with the taper angle to make the diameter further gradually smaller than the small diameter part of the first tapered part 61 toward the front end atθ2. The relation of (the taper angleθ1> the taper angleθ2 is set between the taper angleθ1 of the first tapered part 61 and the taper angleθ2 of the second taper angleθ2 of the second tapered part 62. Namely, the second tapered part 62 is formed more acutely than the first tapered part 61.
