PROBLEM TO BE SOLVED: To generate accurate and uniform surgical operation records in a short time through easy operation by recording an arbitrary item to be written in a chart read out of a file in the chart, and selecting and recording an arbitrary opinion in an opinion list recorded in an opinion file associatively with the chart. SOLUTION: Doctor check files 25 are provided by surgical operation names and contents corresponding to surgical operation items are recorded in e files 25. The doctor check file 25 corresponding to a selected item file 23 is read out and displayed on a display 6 together with a drawing read out of a drawing file 24. A doctor when generating a total opinion calls the opinion list file 26 and selects proper contents in it. When surgical operation contents are inputted, writing to the displayed chart is possible. For opinion input, one of opinion list files which are displayed is selected or handwritten character input is executed. |
