PROBLEM TO BE SOLVED: To form perforations at a low cost without decreasing the yield due to later steps or the like by forming perforations before folding a sealing folding part in a bag making machine for forming an envelope. SOLUTION: Envelope raw sheets 1 each provided with a window and adhesive at the top are disposed on a magazine rack 15 of the bag making machine having a perforation forming unit, and supplied one by one onto a conveying base 23 by a sheet supply roll 16. The sheet 1 supplied onto the base 23 is set to a gap between a perforation forming roll 18 of the perforation forming unit and an impression cylinder roll 19 at predetermined timing by a conveying roll 21. And, perforations are formed at a sealing folding part on an outer periphery of the roll 18 by a perforation forming cutter 20 provided perpendicularly to the direction of rotation while being sandwiched between the roll 18 and the roll 19 and conveyed.
