PROBLEM TO BE SOLVED: To make it possible to easily measure the distance between an optical element and the front end of an optical fiber by providing the periphery of a guide groove or an optical element mounting part with position measuring marks for the optical fiber or position measuring marks for the optical element capable of measuring the misalignment quantity of the fixing position of the optical element and having a correlation with the position measuring marks for the optical fiber. SOLUTION: The periphery of the guide groove 20 and the optical element mounting part 14 is provided with the position measuring marks 30 for the optical fiber capable of measuring the positional misalignment quantity of the front end fixing position of the optical fiber 11 and the position measuring marks 30 for the optical element capable of measuring the misalignment quantity of the fixing position of the semiconductor laser element 13 and having the correlation with the position measuring marks 30 for the optical fiber. In addition, both edge sides of the guide groove 20 are provided with reference marks 40 which have the correlation with the optical element mounting part 14 and measure the positional misalignment quantity in the width direction of the guide groove 20.
