The form consists of pages (13) designed to receive variable inscriptions (8) and covered with transparent film (11) to protect the inscriptions. Each page consists of a base layer (2) and a film (3) made from one or more printed layers of adhesive and/or varnish, with a free surface to receive the inscriptions (8). The film (3) comprises a first adhesive layer which is heat reactivated and based on an acrylic polyurethane copolymer in aqueous dispersion and applied by cold silk screen printing or transferred by dry pressing to one face of the protective film destined to be applied to the document. |
