PROBLEM TO BE SOLVED: To reduce the weight and the size of the device by arranging the silicon substrate, which forms main and suboptical waveguides on a silicon base, and adhesively fixing the photodetector in which the laser light source located on the tip of the waveguides, a monitor light receiving element located in the vicinity of the source, the mirror located on the other tip and the light receiving element for error detection are arranged in the input output tips of the suboptical waveguide. SOLUTION: In the one tip of an optical waveguide 4 formed along a silicon substrate 1', a laser diode 3 is arranged on a silicon base 1 and a monitor photodiode 2 is arranged on the opposite side of an optical waveguide 4 in the vicinity of the diode 3. Then, an optical filter 5 is buried in the middle of the waveguide 4, a mirror 6 is arranged on the other end of the waveguide 4 and the one end of a diffraction grating 7, on which a lens 8 is adhesively fixed, is adhesively fixed on the upper end of the mirror 6. An optical disk 9 is arranged on top of the lens 8. Moreover, an optical waveguide 4' is branched from the filter 5 approximately normal to the waveguide 4'.
